What tool do you use to create initial neighbor relations for a new site?
Is there any free tool, maybe some VBA script which can help in this task?

From free tools, I can recommend only MCOM 4.2. May be someone else know better.

You can start with something very basic based on distance, and add the nearest relations, use a distance calculator.
Use this formula in excel
=1.15191.60960*SQRT((C2-F2)^2+((D2-G2)*COS((C2+F2)*PI()/360))^2)

Where
C2 - Latitude source site
D2 - Longitude source site
F2 - Latitude target site
G2 - Longitude target site
